About The Role

We are seeking an experienced Referrals Manager to join our Business Development Team at the Hesley Group.
Reporting to the Head of Adult Business Development, you will be responsible for effectively managing and overseeing the progress of the adult referral process, including school leaver/potential internal transfers coming into adult services for both residential and supported living placements, across the business.

You will work together with social care and education colleagues in the management of referrals to the post-19 college provision and adult residential living services and effectively identify where services can be offered.

It will be essential that accurate and up to date records are maintained relating to referrals coming into the business, as well as maintaining an overview of the ongoing completion of assessments and an up to date overview of current vacancies across the business, to ensure effective admissions are achieved.

You will be required to produce accurate and timely progress reports to the Head of Adult Business Development as well as operationally agreed transition plans for appropriate costing and submission. 



About You

It is essential that you have an understanding of associated needs relating to the individuals requiring our services – Autism, learning disabilities and other complex needs as well as the Diploma level 5 Health & Social Care or equivalent and at least 2 years experience working on or as part of a referrals team

You will be a well organised individual who is able to manage their workload effectively whilst not being afraid to seek help and support when required.  The successful candidate will be able to demonstrate both a commitment to the role and the individuals coming into our services as well as a keen desire to ensure people are being offered the best possible opportunities.  It is essential that you possess excellent verbal communication skills and demonstrate a high degree of honesty and integrity.
